This model is the Matchbox Yesteryear Y12-3 Ford Model T Van (1912) in brown Rowntrees Cocoa livery. This Code 3 items has been made as a mobile tea shop and comes packed in its original straw coloured box.
The model has no opening parts. The vehicle roof is white, the seats are black plastic. The model has been specially modified by cutting off most of the rear body and replacing it with a new body, possibly of light brass, designed to look like an old mobile shop/canteen, complete with serving hatch and tea urn. The roof has also been replaced with a new one piece curved roof. The decals possibly represent a GWR mobile canteen as the front doors carry a vehicle number "16", "GWR" under the left side window, and "Rowntrees Cocoa" advertising on the rear left body. The right rear body has been opened to form a counter with counter-top and tea urn. Below the right side window is a sign reading "TEAS, ~~~ 2d, ~~~ 3d, ~~~ 5d". There is no glazing.

Other Comments/Identifying Features: The model has a code 3 rear body, and no glazing. The base plate is held in place with two machined rivets, has no base hole at the rear of the body, and has a Lesney base without Y12.
This Code 3 model has been assembled very well by an unknown/un-attributed modeler. It has a stated scale of 1:35. I have seen other identical examples of this model so it is not a "one-off" item.
